I am looking for forms to waive parental rights but no adoption is involved. Single parent & ex-boyfriend only
I reside in Florida and am looking for forms to terminate parental rights for my child. I am a single parent and the father lives in Texas and has nothing other than child support to do with my child.
There is no adoption involved; I only want to terminate his rights to my child who has my last name.

If you terminate his rights, he will no longer have to pay support.
The Department of Health and Human Resources (or whatever they are called in Florida) cold help point you in the right direction. At some point an attorney might have to become involved, even if it’s just for oversight. Most often these cases have to go before a judge. So someone will have to do some traveling.
I believe you need to get an attorney. The other girl is right once terminated he is totally out of the picture, no money nothing. If you do not care about that, an attorney is the safe route to make sure it is done right. I am sure their are some that are not going to charge you. Have you gone through the state, maybe call and ask someone there in human services, about attorneys, or if you can afford one yourself. I would not just get a form and fill it out and file it, I would do it the right way, this is your child you are talking about. Good Luck!

Contact a lawyer.
Why dont you want to continue with child support.
You could try a name change if you want. Father will probably consent to it or default on responding.
You just have to show that the name change is in the best interest of the child.
Court typically do not want to terminate if there is no other child support/adoption.
There are lots of adoption support groups online that can help you. Never Give Up!
